Dr. Ho is closing her private practice and will join the Baylor Scott & White system. The last day that she will see patients at her practice, Raintree Medical Group in Allen will be October 31, 2024. She will continue to provide medical care for you at the practice prior to this closing date.
She will start seeing patients on December 5, 2024 at Baylor Scott & White Family Medicine Clinic in Rockwall.

It is important that you continue with primary medical care, so please select one of the following-
Option I: You wish to continue under Dr. Ho’s care at the Baylor Scott & White Family Medicine practice in Rockwall.
Option II: You wish to place yourself under the care of another primary care physician. I have attached a list of local physicians and their contact info for reference.
Option III: If you are in a managed care situation or narrow network, you may need to contact your plan’s administrator for a referral in your network.
Option IV: The patient’s age is 21 yrs or younger, and you wish to transfer his/her care to the pediatric providers at Raintree Healthcare.
The email notice will include a form to authorize the release of your medical records to you, Dr. Ho’s new practice, or the healthcare provider of your choice. Please complete and return this as soon as possible.
Once the office is closed, your medical records will continue to be stored by the current vendor of our electronic medical record, TriMed Technologies. Future medical record requests can be sent to records@raintree-healthcare.com. Requests will be processed as per time limitations dictated by state law retention requirements.
